National Anthem Contest, UW Obstetrics and Gynecology

By entering, the contestant certifies that he/she is available and able to attend the Mini-Marathon on August 20, 2016 at 7:00 AM. Mark your calendar and set your alarm.

Please provide a link to your audition on YouTube, Viemo or other popular video hosting websites. Videos may not be longer than 2 minutes and entrants must state their name, age, and town at the beginning of the video.

Entrant must be 18 years of age or older on the date of entry. If entrant is under the age of 18, he or she must obtain parental consent to submit a video.

Eligibility:: Entrant must be 18 years of age or older on the date of entry. If entrant is under the age of 18, he or she must obtain parental consent to submit a video. Employees of the Department and contest sponsors, their respective parent entities, subsidiaries, affiliated companies and advertising and promotion agencies and their immediate family are NOT eligible. By entering, the contestant certifies that he/she is available and willing to attend the Mini-Marathon on August 20th, 2016 at 7:00 AM.

How to Enter: The Entry Period will begin on July 1st, 2016 and end on July 31st, 2016. The voting period begins on August 1st, 2016 and ends on August 6th, 2016.

Internet Entry: Enter by visiting the UW-Madison Ob-Gyn website, completing all of the required information, and submitting one original video via a YouTube link in which the entrant sings the National Anthem. Each video submission must:
a. be an original video created by the contestant
b. be no longer than 2 minutes
c. include the entrants name, age, and town at the beginning of the video
d. be uploaded to YouTube and submitted to the Department website via the YouTube link

The Department will not be responsible for incomplete, lost, late, misdirected, or illegible entries. All entries become property of the UW Department of Ob-Gyn. Contestants acknowledge and agree that the Department and its affiliates shall have the right to edit, adapt, modify, reproduce, publish, promote, broadcast, or otherwise display or use entries in any way it sees fit without limitation or compensation to contestants. All entries may be posted on the Department’s websites or social media profiles, or shared by contest affiliates and sponsors.

Winner Selection: All eligible entries will be reviewed by staff members of the Department of Ob-Gyn and posted to the Department’s Facebook page. Facebook likes from August 1st, 2016 through August 6th, 2016 will count as ¼ of the final vote, while ¾ of the voting will come from Ob-Gyn staff.

Prize: The winner will get to sing The National Anthem at the Madison Mini-Marathon on August 20th, 2016. Winner must attend the event at 7:00am. Winner may also receive Madison-Mini Madison apparel and/or other prizes.

Winner Notification:The winner will be notified by a staff member from the Department of Ob-Gyn on Thursday, August 11, 2016. The winner must respond to the winner notification within the time period specified in the email notification or phone message. Failure to respond within such time period or return of email prize notification as undeliverable may result in selection of a substitute winner from among all remaining eligible entries. Winner must present valid photo identification. Winner may waive their right to receive prizes.

Copyright: By entering the contest, each contestant grants to Department of Ob-Gyn an exclusive, royalty-free and irrevocable right and license to publish, print, edit or otherwise use the contestant's submitted entry, including any accompanying video or description, in whole or in part, for any purpose and in any manner or media (including, without limitation, the Internet) throughout the world in perpetuity, and to license others to do so, all without limitation or further compensation. Each contestant further agrees that if his/her entry is selected by the Department as the winning entry, he/she will sign any additional license or release that the Department may require.
